Here's the pattern that inspired the Chaos Locust from the foam fly swap.

 

 

This one looks like someone just put a foam fly on the hook the way you might put a grasshopper on. There is a nice story about its effectiveness along with how to tie it at:

now that is what i call a simple tie. i would put some dubbing on the bare hook shank as it looks to naked. rich

Rich, I agree, I might also take a long dry saddle hackle and palmer it tightly from the back to the front, taking care of the bare hook and adding a zillion legs.
